THE DAXXIFY^® DREAM GETAWAY SWEEPSTAKES

OFFICIAL RULES

1. Sweepstakes Timing: The DAXXIFY® Dream Getaway Sweepstakes (the “Sweepstakes” begins at 8:00 a.m. Pacific Time (“PT”) on September 15, 2025 and ends at 11:59 p.m. PT on December 31, 2025 (the “Sweepstakes Period”).
2. Eligibility: The Sweepstakes is open only to legal residents of the 50 United States and the District of Columbia who are 21 years of age or older as date of entry. Employees of Revance Therapeutics, Inc. (the “Sponsor”), their respective parents, subsidiaries, divisions, affiliates, suppliers, distributors and advertising, promotional and judging agencies, including Don Jagoda Associates, Inc. (“Administrator”) and their immediate family members (spouses, parents, children, and siblings and their spouses) and household members of each (whether related or not), are not eligible to participate or win. Void where prohibited by law. All federal, state, and local laws and regulations apply.
3. HERE’S HOW IT WORKS: During the Sweepstakes Period, entrants can visit https://daxxify.com/lp/dream-getaway or https://daxxify.com/lp/dream-getaway-sweeps (“Website”) and follow the instructions to complete and submit the online entry form, including answering a series of DAXXIFY related survey questions to earn one (1) entry into the Sweepstakes (“Entry”). Limit one (1) Entry per person/email address. Any attempt by any entrant to enter using multiple/different email addresses, mailing addresses, identities, registrations and logins, or any other methods will void such Entries and that entrant may be disqualified. Use of any automated or computer system to participate online is prohibited and will result in disqualification. Normal Internet/phone access and data/usage charges imposed by your online/phone service may apply. Not responsible for lost, late, or illegible Entries.
4. Random Drawing: Five (5) Grand Prize winners will be selected in a random drawing on or about January 5, 2026 from among all eligible Entries received by Administrator, whose decisions are final on all matters relating to the drawing. Odds of winning are dependent upon the total number of eligible Entries received during the Sweepstakes Period.
5. Winner Notification: The potential Grand Prize winners will be required to sign and return a Declaration of Eligibility and Liability Release and except where prohibited, publicity release and Assignment of Rights, as applicable (“Declaration”) within five (5) days of notification in order to be confirmed as a winner. If a potential winner fails to return the completed Declaration within five (5) days or Declaration is returned as undeliverable, the entrant shall be deemed to be ineligible and the prize will be forfeited and an alternate potential winner will be selected. Declarations will be sent via email. Return of any prize notification as undeliverable may result in disqualification and alternate selection. Each travel companion must be 21 years of age or older at time of travel and must execute a Liability/Publicity Release (where legal) prior to issuance of travel documents.
6. Prizes and Approximate Retail Value (“ARV”): Five (5) Grand Prizes: A 5-day/4-night trip for winner and one (1) travel companion to their choice of destination to either Miami, Florida or Los Angeles, California for the winner to get one (1) DAXXIFY treatment valued up to $1,200 (including costs of services) with an expert injector (“Trip”). Trip includes round-trip economy class air transportation to winners’ choice of destination, ground transportation to/from the airport and hotel (awarded in the form of an Uber gift card), and $1,000 awarded as a check to winner only for meals, incidentals and to help offset income taxes. The ARV of the Grand Prize is $5,650. Sponsor will provide the winner with injector options located in their chosen destination and winner will select their preferred injector, based on availability. If injector determines that a winner is not eligible or does not qualify for the DAXXIFY treatment, that portion of the prize will be substituted with a prize of equal or greater value.
   
   Limit one (1) prize per person/email address. Sponsor reserves the right to substitute a prize of equal or greater value if any advertised prize (or portion of prize) is unavailable for any reason. No substitution or transfer of prize by winner is permitted, other than as described above. Income taxes related to the Grand Prizes are the sole responsibility of the winners. Winners will be issued a 1099 tax form for the actual retail value of prize for the year that the prize is awarded. Trips must be taken within one (1) year of winner notification and restrictions/blackout dates may apply. Sponsor will determine airline and flight itinerary in its sole discretion. Airline carrier's regulations and conditions apply. No refund or compensation will be made in the event of the cancellation or delay of any flight. Travel and accommodations are subject to availability. Travel is subject to the terms and conditions set forth in this Sweepstakes, and those set forth by the Sponsor’s airline carrier of choice as detailed in the passenger ticket contract. Winners and travel companions agree to follow any applicable COVID or other protocols that may be in place at time of travel. Trips may not be combined with any other offer and travel may not qualify for frequent flyer miles. Travel companions must travel on same itinerary and at the same time as the respective winner. The winners and travel companions must possess valid government-issued photo identification (e.g., REAL ID-compliant driver’s license or REAL ID-compliant non-driver ID, passport, passport card, etc.) in order to travel. Travel restrictions, conditions and limitations may apply. Sponsor will not replace any lost, mutilated, or stolen tickets, travel vouchers or certificates. Actual value may vary based on airfare fluctuations and distance between departure and destination. All other expenses and costs, not expressly listed above, including, but not limited to, taxes, insurance, telephone charges, room service and laundry services, personal expenses, gratuities, entertainment, transfers, incidental charges, or any other items not specifically described in these Official Rules, any transportation not mentioned within such prize descriptions and all expenses for any of the foregoing are the sole responsibility of winners.
7. General Rules: By participating in the Sweepstakes, entrants agree to be bound by these Official Rules and the decisions of the Sponsor/Administrator. In the event there is a discrepancy or inconsistency between disclosures or other statements contained in any Sweepstakes materials and the terms and conditions of the Official Rules, the Official Rules shall prevail, govern and control. Entries become the property of the Sponsor. The Sponsor and its agencies are not responsible for technical, hardware, software or telephone malfunctions of any kind, lost or unavailable network connections, or failed, incorrect, incomplete, inaccurate, garbled or delayed electronic communications caused by the user or by any of the equipment or programming associated with or utilized in the website or the Sweepstakes or by any human error which may occur in the processing of the entries in the Sweepstakes or for any liability for damage to any computer system resulting from participation in, accessing or downloading information in connection with the Website or the Sweepstakes. In the event of a dispute as to any Website entry, the authorized account holder of the email address used at time of entry will be deemed to be the entrant. Winners may be required to show proof of being the authorized account holder of the email address provided at time of entry. Proof of entering does not constitute proof of delivery or receipt of such information. Sponsor (as identified in these Official Rules) is not responsible for the failure of any email or entry to be received by Sponsor on account of technical problems or congestion on the internet or at any website. The Sponsor reserves the right at its sole discretion to disqualify any individual that tampers or attempts to tamper with the entry process or the operation of the Sweepstakes or website; violates the Official Rules; or acts in an unsportsmanlike or disruptive manner, or with intent to annoy, abuse, threaten or harass any other person. ANY ATTEMPT BY ANY PERSON TO DELIBERATELY UNDERMINE THE LEGITIMATE OPERATION OF THE SWEEPSTAKES OR WEBSITE MAY BE A VIOLATION OF CRIMINAL AND CIVIL LAW, AND, SHOULD SUCH AN ATTEMPT BE MADE, SPONSOR RESERVES THE RIGHT TO SEEK DAMAGES FROM ANY SUCH PERSON TO THE FULLEST EXTENT PERMITTED BY LAW. Sponsor's failure to enforce any term of these Official Rules shall not constitute a waiver of that provision. If, for any reason, the Sweepstakes is not capable of running as planned, including infection by computer virus, bugs, tampering, unauthorized intervention, fraud, technical failures, or any other causes beyond the reasonable control of the Sponsor which corrupt or affect the administration, security, fairness, integrity or proper conduct of the Sweepstakes or Website, then the Sponsor reserves the right at its sole discretion to cancel the Sweepstakes and select the winners from among all eligible entries received prior to cancellation. By entering the Sweepstakes, you hereby represent and warrant that you have read these Official Rules and are fully familiar with its contents.
8. Release and Limitations of Liability/Publicity Release: By participating in the Sweepstakes, the entrant agrees that Sponsor, Administrator, their respective parents, subsidiaries, affiliated companies, agents, advertising and promotion agencies (collectively, “Sweepstakes Indemnities”) and each party’s respective affiliates, officers, directors, agents, and employees will have no liability or responsibility for any claim arising in connection with participation in the Sweepstakes or the awarding of prizes. The winners/travel companions assume all liability for any injury or damage caused, or claimed to be caused, by participation in the Sweepstakes or use or redemption of the prize. By entering the Sweepstakes, you acknowledge that you, not Sponsor, are responsible for the contents of any material submitted in relation to the Sweepstakes. Entrants waive the right to claim any damages whatsoever, including, but not limited to, punitive, consequential, direct, or indirect damages. You hereby agree that the Sweepstakes Indemnities have the right to use your name, likeness or any other rights granted to Sponsor, including any claims, including but not limited to, trademark, copyright, or other intellectual property rights, right of publicity, right of privacy or defamation in Sponsor’s sole discretion.
9. Governing Law: The Sweepstakes and the Official Rules shall be exclusively governed by and construed in accordance with the laws of the State of Tennessee, without regard to conflicts of law provisions. Entrants submit to exclusive personal jurisdiction in Tennessee and agree that any dispute shall be brought in the state or federal courts in Tennessee. You agree that: (a) any and all disputes, claims and causes of action arising out of, or connected with the Sweepstakes or any prizes awarded shall be resolved individually, without resort to any form of class action, and exclusively by the appropriate court located in the State of Tennessee; (b) any and all claims, judgments, and awards shall be limited to actual out-of-pocket costs incurred, including costs associated with entering the Sweepstakes, but in no event attorney’s fees; and (c) notwithstanding the foregoing, you waive the right to claim any damages, whatsoever, including, but not limited to, punitive, consequential, direct or indirect damages.
